Information needed

• Application
• Payroll Cost Estimates
• Customer Identification Information*
• Copy of 2019 Business Tax Return……2018 if 2019 isn’t filed
• Other Documents As Required By SBA

• Driver’s License and SSN for all signers of a company
• Articles of Organization and Operating Agreement
• Articles of Incorporation and Bylaws

Once you have completed the application and gathered all of the required documentation, you will be ready to go forward
